About Us
We at Adapt Integrated Health Care are committed to reducing barriers to care and ensuring that every person can realize their highest possible level of health, regardless of ability to pay.
We are opening doors to health and recovery for more people by offering fully-integrated care—primary care, mental health care, and substance use treatment. This means that our patients have greater access to a multidisciplinary network of providers to address their unique healthcare concerns and needs.
This health center receives U.S. Department of Health and Human Services (HSS) funding and has U.S. Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
Our Commitment to Health Equity
Adapt Integrated Health Care is committed to being an organization that is safe, supportive and welcoming for all. Health equity and equitable access to the highest quality primary care and behavioral health care is at the heart of what we do each and every day.
