The Community Support Team at Adapt Integrated Health Care provides a number of services to improve quality of life for individuals living with mental health disorders. Our Clinical Case Management services are designed to help our clients develop the skills and community supports they need to achieve recovery, self-sufficiency, stability and an overall improved quality of life.
Our caring staff works with clients to identify the services and resources needed to support successful transition to independent living—including, assistance with health insurance, SNAP, Social Security benefits, housing and utility assistance. Clinical Case Management services are available by referral from a Adapt Integrated Health Care treatment provider.
