What is Healthcare Planning?

Healthcare Planning is the process of thinking through the care that you want before you need it. This is often called Advance Care Planning (ACP).

ACP allows you to describe the kind of care you want to receive and who should speak for you in an emergency. Your doctors or loved ones don’t need to guess if you can’t tell them yourself. Your preferences for care can be documented in what are called Advance Directives.

Umpqua Health has made Empower Advance Care Planning (an Iris Healthcare platform) available to all Douglas County residents over the age of 18 with the goal of improving population health and health care quality.
