Case Management

Our Clinical Case Management services are designed to help our clients develop the skills and community supports they need to achieve recovery, self-sufficiency, stability and an overall improved quality of life.

Our caring staff works with clients to identify the services and resources needed to support successful transition to independent living—including, assistance with health insurance, SNAP, Social Security benefits, housing and utility assistance. Clinical Case Management services are available by referral from a Adapt provider.

Our Services

  • Behavioral Health Case Management: Ongoing support for clients engaged in Adapt’s mental health programs.
  • Community Resource Navigation: Referrals to housing assistance, food programs, employment services, healthcare, and other essential services.
  • NEW One-Time Emergency Case Management (Open Access): Walk-in service for immediate support and referrals to help meet urgent social and economic needs

Why Case Management Matters

Studies show that effective case management can reduce service gaps, improve quality of life, lower hospitalization rates, and address barriers related to social determinants of health. The Open Access Case Management program was developed in response to a growing need for immediate case management services outside of traditional mental health referral systems.
