Adapt is informing you of a recent information security incident that may have involved some limited personal information of our Mobile Crisis contacts.
On March 11, 2026, we identified that on February 25th an employee sent a limited report of Mobile Crisis clinical service hours that inadvertently included limited protected health information (PHI) including name, medical record number (MRN), date of birth and basic service type and date of service. Upon discovery, we took immediate action to secure this disclosure and request and confirm its removal and destruction. Actions were taken to prevent future occurrences.
