Adapt Community Terms of Service
General Terms
By opting in to receive communications from Adapt Integrated Health Care (“Adapt,” “we,” “us,” or “our”) via SMS, and email, you agree to the following Terms and Conditions. These terms apply to both email and SMS subscriptions delivered through our third-party service provider, Mailchimp. These communications may include event invitations, press releases, newsletters, closure notices, surveys, and other updates about Adapt services and programs.
Consent to Receive Communications
When you opt in to the Adapt Community, you will receive an initial message to confirm your signup. By providing your email address or mobile number, you consent to receive recurring informational and promotional communications from Adapt via SMS, and/or email. Your continued use of our SMS and/or email programs constitutes your continued agreement to these Terms and our Privacy Policy. You acknowledge that these communications may be sent from Mailchimp, or other similar third-party communication service providers.
You will never automatically be subscribed to receive communications. You must opt-in to subscribe.
No Purchase Required
Your consent to subscribe is voluntary and is not a condition of purchase of any Adapt service. Standard message and data rates may apply for SMS communications. Your wireless provider is not responsible for delayed or undelivered messages.
Message Frequency
Message frequency varies and may increase or decrease based on the type of content you request, and your engagement with the Adapt Community. Adapt reserves the right to adjust the frequency of communications based on business needs and communication priorities or to change the sending domain, phone number, or short code used to deliver messages.
Opt-Out & Help
SMS Opt-Out. You may opt-out your SMS subscription at any time by replying “STOP” to any message you receive from Adapt. After you send STOP, you will receive one final message confirming your unsubscribe request, and no further SMS messages will be sent unless you opt in again.
Email Opt-Out. For email communications, you may unsubscribe at any time by clicking the “unsubscribe” link included at the bottom of every email. Opting out will remove you from receiving future communications via the respective channel but will not affect any ongoing care services or relationships with Adapt.
Help. If you require assistance with SMS messaging, you may reply HELP at any time and instructions will be provided. For additional support related to either email or SMS communications, please contact us at communications@adaptoregon.org.
Re-Enrollment. If you wish to continue receiving messages after unsubscribing, you must re-register through the Adapt Community.
Message and Data Rates
Standard message and data rates may apply to communications sent by Adapt via SMS. These charges depend on your mobile carrier and plan. Adapt is not responsible for any charges incurred as a result of receiving SMS messages.
Privacy Policy
Your privacy is important to us. By opting in, you agree that Adapt may collect and use your personal information in accordance with our Privacy Policy, which governs how we collect, use, and protect your personal data. We collect and process personal information through designated service providers, including Google Analytics and Mailchimp, solely for the purposes described in our privacy policy. Service Limitations and Disclaimers
Adapt makes no guarantees regarding the availability or performance of SMS, or email communications. We are not responsible for delays or messages failures due to network issues, carrier problems, or other technical disruptions. Messages may be delayed or not delivered due to factors beyond our control, including device compatibility or issues with your mobile carrier.
Changes to Contact Information
If you change or transfer your mobile number, you are responsible for notifying Adapt or unsubscribing from your old number before reassignment. Adapt is not liable for continued messaging if you fail to notify us of such changes.
Changes to Terms
Adapt reserves the right to modify or terminate its communications program at any time. Any changes to these Terms & Conditions will become effective immediately upon posting, and your continued subscription following such changes constitutes acceptance of the updated terms.
