Starting January 1, 2025, Adapt Integrated Health Care is streamlining its informed consent process. With changes to Substance Use Disorder (SUD) Privacy Laws, we are now offering the option to sign a single consent form. This new process enhances communication and coordination between you and your healthcare providers.

What Does Single Consent Mean for You?

By signing the single consent form, you allow Adapt to:

  • Communicate with your healthcare provider.
  • Coordinate with your insurance.
  • Collaborate with other healthcare services.
  • Share essential information with the hospital when needed.

This ensures smoother care delivery, improved coordination, and fewer delays.

What Happens Without Single Consent?

Without a signed single consent, we cannot:

  • Share your information with healthcare providers outside of Adapt without a signed release of information.
  • Coordinate with your insurance without explicit approval.
  • Communicate with hospitals except in cases of bona fide emergencies.
  • Collaborate with other healthcare services without additional signed releases.
