The Adapt Diabetes Prevention Program extends from the CDC’s National Diabetes Prevention Program to provide individuals with the support and guidance they need to mitigate the risk of Type 2 diabetes.

With a focus on healthy lifestyle changes, our program is a comprehensive, year-long journey towards better health!

What We Do

Based on the CDC-recognized lifestyle change program, is dedicated to helping you reduce the risk of Type 2 diabetes through:

  • Structured Lifestyle Change: Emphasizing healthy eating and increased physical activity, we help participants aim for a weight reduction of 5 to 7 percent and 150 minutes of weekly activity.
  • Evidence-Based Approach: Leveraging the research-backed strategies that have shown a significant reduction in the risk of developing Type 2 diabetes by 58% overall, and 71% in individuals over 60 years old.
  • Supportive Environment: With a trained lifestyle coach and sub-coordinator, our small group setting ensures personalized attention and mutual support.

Program Details

  • Program Start: Launching on May 7th, aimed at enrolling up to 14 members for this cycle.
  • Confidentiality: Maintaining privacy in compliance with HIPAA regulations, ensuring sensitive health information is protected.
  • Insurance Coverage: The program is covered by Medicare and Medicaid, ensuring accessibility for participants.
  • Ongoing Support: Weekly support sessions from our trained lifestyle coach over the course of the year.
